Replicable circumstances: if the final killcam happens at the same time that "Prisoners have 3 seconds to get weapons and take position" message appears, all the guards spawn in the main cell area at the beginning of the next round (where they would for deathmatch).
It happens more often than one would think with the new LR rules and given that many prisoners with LR have to type their orders.

+1 This always happens when a prisoner takes for ever to come up with his/her Lr.

Andy I already said that this happens with long typed LRs. But whatever, the more input the better?
