I'm not sure if its entirely rare, but sometimes when I pick up an M4 out of the Armory or sometimes off of the ground, it as 0 in the clip and 0 in reserve.

That's because guards or a prisoner has dropped their weapon from it being empty, if it's from armory then it's just you
